SOAP

SOAP /sōp′, es″ō′ā′pē″/ , (in a problem-oriented medical record) abbreviation for subjective, objective, assessment, and plan, the four parts of a written account of the health problem. In taking and charting the patient history and physical examination, a SOAP statement is made for each syndrome, problem, symptom, or diagnosis. Charting by this method is said to be “soaped,” and charts produced by using it are called “soap charts.” See also problem-oriented medical record.
