The electrocardiogram is normal (no delta wave) during sinus rhythm and is not associated with preexcitation, but the patient is prone to paroxysmal supraventricular tachycardia caused by orthodromic circus movement tachycardia. Conduction into the ventricles is normal during atrial fibrillation because the accessory pathway does not conduct in that direction.
