zoster ophthalmicus [Gk, zoster, girdle, ophthalmos, eye] , a herpes infection of the eye and the first division of the trigeminal nerve. ▪ OBSERVATIONS: The infection frequently involves the cornea. There may be lid edema, ciliary and conjunctival involvement, and pain. Keratitis may be severe. Scarring and glaucoma are common sequelae. ▪ INTERVENTIONS: The administration of oral acyclovir or valacyclovir is used to treat the herpes virus. Cool compresses can be applied as a comfort measure. Concurrent infection is treated with broad-spectrum antibiotics. Topical steroids may also be of use. The pain associated with this condition can be significant. Analgesia is frequently required. ▪ PATIENT CARE CONSIDERATIONS: The herpes zoster vaccine can reduce significantly the burden of herpes zoster among older people. Health care providers should discuss and encourage vaccination.
